Note that the first appointment will be for consultation only, and treatment(s) may or may not begin until a later appointment.
It is very important that you complete the following tasks for your appointment with Dr. Bryan Lee.
Download and fill out the forms below and BRING to your doctor’s visit.
Bring your Insurance card, Picture ID card, and Prior Authorization Form (usually if you are with an HMO).
All co-pays, deductibles, and co-insurance will be collected during check-in. We accept cash, ATM, credit cards (Visa, Mastercard, American Express, and Discover), money order/cashier’s check. No personal checks are accepted.
Bring all your medications (including prescription bottles, boxes).
Bring all pertinent medical records (doctor’s notes), radiological reports (ex: X-rays, MRI, CT scan) and lab reports.

Return Patient (Follow-up)

Pre & Post Procedure Instructions
If you are getting a pain procedure (ex: epidural steroid instructions), you were given a PRE-PROCEDURE INSTRUCTION FORM (before the procedure) and a POST-PROCEDURE INSTRUCTION FORM (after the procedure).
